Responsible for providing Telephonic Disease Management including coaching members on disease specific medical/nutritional requirements, signs/symptoms, and avoidance of complications. Position is also responsbile for assessing a member’s readiness to change, utilizing the Prochaska Model, and applying appropriate intervention(s). Coaching includes establishment and follow-up of goals, distribution of educational materials to participants and identification and referral of appropriate members to case management. This position is also responsible for performing data entry of information into designated disease management software programs.

Qualifications/Skills required: Candidates must be a current, unrestricted Texas Licensed Dietitian and a Certified Diabetes Educator with a minimum of 3 years recent experience practicing as a Certified Diabetes Educator. Candidates should also have at least 3 years current dietitian experience with diabetes and cardiovascular populations. Disease Management and/or Behavioral Health experience is preferred. Candidates must have knowledge of available disease specific resources and advanced knowledge of endocrine and cardiovascular systems. CCM (McKesson Software) experience is highly preferred and one year of Microsoft Office experience is required.
