Title:Social Worker
Job Summary: The responsibilities of the Social Worker are to provide social casework as an entity where appropriate to individual and families receiving care at this facility, to coordinate the delivery of services to meet patient’s discharge needs, to be capable and comfortable offering alternatives in patient care (Home Health, SNF, ALF, ect.), to facilitate timely and appropriate discharge from the hospital and to provide documentation of all functions as required by regulatory agencies. This position requires knowledge of Case Management policies and procedures and State and Federal regulations. Knowledge of community resources and discharge planning, along with the technical skills necessary to demonstrate alternative care programs to patients and their families. Frequent interactive, interpretive and consultative associations are inherent to this position. The ability to retrieve, communicate, and present data and information verbally and in written form if required. Strong interpersonal skills and the ability to communicate effectively with patients, families, nursing staff, physicians, associations, and community resource liaison are inherent to this position. A professional appearance is essential. The Social Worker must possess knowledge of Universal Precautions. The ability to maintain confidentiality of information is required. Participate in Patient Care Conferences identifying social problems, the severity, interrelatedness to the medical situation, as well as assessing a family’s strengths and weaknesses. Assess the social and emotional factors as they relate to the patient’s illness and family dynamics.
Requirements: - Master’s in Social Work (MSW)- Minimum of two years experience in Medical Social Work, preferably in a medical setting with the ability to work with Neonates, Pediatrics, Adolescents and Adults- Current LCSW license - Participation in Continuing Education- Knowledge of Federal and State guidelines and requirements: Health Care District, Medicare, Medicaid and Department of Children and Families- Must possess excellent interpersonal and communication skills- Should have an aptitude and experience in counseling- Must have the ability to exercise sound judgment
