Job Details:

The Case Manager works as part of a multidisciplinary team across the continuum of care to initiate, implement, and coordinate all aspects of the utilization management (UM) and discharge planning function from pre-admission through post-discharge for an assigned patient population. The Case Manager’s primary focus is on assessment of medical severity of illness of patients, and intensity and appropriateness of medical services required during an inpatient stay. Additional functions include planning, implementation, and evaluation of plans of patient care, and ongoing concurrent monitoring of appropriateness of that care, in conjunction with hospital and community resources. Bachelor’s degree in nursing or related health care field preferred. Minimum of two years clinical experience and basic computer skills required. Broad clinical background in a variety of settings, and prior Utilization/Case Management experience preferred. Current, valid California nursing license.
